Community Assistance


“Our Family was Selected to
…participate in the family wellness initiative. We were facing homelessness, and staying with relatives while I was pregnant with our third child. The case managers helped us identify and move toward our goals, including finding appropriate housing for our family and helping my husband find a better job.”
The Family Wellness Initiative is a grant-funded program that provides intensive case management services through a workshop cohort model. Participating families receive 16 hours of education in parenting, financial literacy, overcoming setbacks, and goal setting.
Financial Assistance
In 2024, we assisted 5,422 Lumpkin County residents with community navigation services including:
- Rent/Utility assistance ($70,000 in assistance in 2024) in 2024, 109 families received assistance for the first time.
- Transportation (we transported 511 individuals in 2024) to medical, employment or other essential appointments)

Intensive Case Management
Our case managers assist with everything from vital records recovery to housing and job assistance, applications, referrals, and social connections. Our case managers conduct home visits, assessments, and host social gatherings for clients to spend time with one another and socialize.
Voucher Program
Through our community navigation services, we provide clients with vouchers to purchase fuel, or to shop for essentials in our thrift store. In 2024, we provided $7440 in fuel vouchers to local families and individuals to help them attend job interviews, medical appointments or other essential trips.


Opportunity Lumpkin…
is a grant-funded initiative that allows us to operate Buster – our Opportunity Bus- and to facilitate our community mentor program. In 2024, our mentoring program volunteers provided 740 hours of service and drove 3,966 miles in support of identified clients who participated in the mentoring program. This program is made possible by our involvement with Georgia State University as a Health Opportunity Pilot site.

This sweet little boy was gifted a bed after he and his mom moved out of a hotel and into their own house- his mom was part of the Opportunity Lumpkin mentoring program.
